Booking Request – One-on-One Training

This form will allow potential clients to submit a booking request for a particular service. A request for payment will be sent only after I have reviewed the booking request and confirmed I am able to fulfill the request. All requests using this form are tentative until, 1) I have confirmed I can fulfill the request, 2) payment has been requested and received, and 3) all requested registration information has been completed.

Dog Training Agreement

This agreement between CLIENT and PRUDENT K9 TRAINING, LLC pertains to dog training for CLIENT’s DOG (hereinafter referred to solely as “DOG”). PRUDENT K9 TRAINING, LLC agrees to provide training for DOG at 8225 SMITHFIELD AVE., SPRINGFIELD, VA 22152 (hereinafter referred to as “FACILITY”). The extent of skills and behaviors addressed in dog training provided is based upon dog training package purchased by CLIENT.

PRUDENT K9 TRAINING, LLC will provide dog training only after payment is received in full. Payment is only accepted via the online booking system on prudentk9training.com. Payment is 100% nonrefundable.

PRUDENT K9 TRAINING, LLC will make every reasonable effort to train DOG to meet CLIENT’s desired goals for the DOG’s behavior within the scope of the dog training package purchased. However, PRUDENT K9 TRAINING, LLC cannot guarantee DOG’s future performance or behavior as a result of providing training. CLIENT understands that without practicing and reinforcing these behaviors at home, they will naturally become less reliable over time.

PRUDENT K9 TRAINING, LLC will endeavor to create as safe an environment as possible for the training of DOG and will offer only responsible training and training instructions. PRUDENT K9 TRAINING, LLC agrees to exercise due and reasonable care of DOG while at FACILITY. However, PRUDENT K9 TRAINING, LLC is not liable for loss or damage from disease, death, running away, theft, fire, injury to persons, other dogs, or property by DOG during or after the term of this Agreement. CLIENT agrees to pay all resulting losses and damages suffered or incurred, and to defend and indemnify PRUDENT K9 TRAINING, LLC from any resulting claims, demands, lawsuits, losses, costs or expenses, including attorney fees. PRUDENT K9 TRAINING, LLC will only be held liable for negligent or reckless actions by PRUDENT K9 TRAINING, LLC employees.

CLIENT understands the inherent risks of owning a dog, including but not limited to the risk of dog bites to people or animals, and consequently CLIENT is and will remain responsible for the actions of DOG at all times. CLIENT agrees to indemnify and hold harmless PRUDENT K9 TRAINING, LLC of any and all claims of injury, expense, costs, or damages caused by DOG.

CLIENT authorizes PRUDENT K9 TRAINING, LLC to take DOG into public spaces during his/her stay for the purpose of dog training.

CLIENT understands that the recommendation by PRUDENT K9 TRAINING, LLC of any other product or service is not a guarantee of CLIENT satisfaction with that product or service.

CLIENT grants permission to PRUDENT K9 TRAINING, LLC and its legal representatives and assigns, the irrevocable and unrestricted right to use, publish and broadcast CLIENT's and/or DOG’s photograph(s), likeness and voice, including via video, for trade, advertising, and any other purpose and in any manner and medium; and to alter the same without restriction. CLIENT hereby release PRUDENT K9 TRAINING, LLC and its legal representatives and assigns from all claims and liability relating to said photograph(s), video(s), likeness and voice.

NOTICE TO ALL BOARDING CLIENTS

THE BOARDING OF ANIMALS IS SUBJECT TO ARTICLE 4 (§ 3.2-6518 ET SEQ.) OF CHAPTER 65 OF TITLE 3.2. IF YOUR ANIMAL BECOMES ILL OR INJURED WHILE IN THE CUSTODY OF THE BOARDING ESTABLISHMENT, THE BOARDING ESTABLISHMENT SHALL PROVIDE THE ANIMAL WITH EMERGENCY VETERINARY TREATMENT FOR THE ILLNESS OR INJURY

THE CONSUMER SHALL BEAR THE REASONABLE AND NECESSARY COSTS OF EMERGENCY VETERINARY TREATMENT FOR ANY ILLNESS OR INJURY OCCURING WHILE THE ANIMAL IS IN THE CUSTODY OF THE BOARDING ESTABLISHMENT. THE BOARDING ESTABLISHMENT SHALL BEAR THE EXPENSES OF VETERINARY TREATMENT FOR ANY INJURY THE ANIMAL SUSTAINS WHILE AT THE BOARDING ESTABLISHMENT IF THE INJURY RESULTED FROM THE ESTABLISHMENT’S FAILURE, WHETHER ACCIDENTAL OR INTENTIONAL, TO PROVIDE THE CARE REQUIRED BY § 3.2-6503. BOARDING ESTABLISHMENTS SHALL NOT BE REQUIRED TO BEAR THE COST OF VET CARE FOR INJURIES RESULTING FROM THE ANIMAL’S SELFMUTILATION.
